Complete this form after you’ve registered with the enrichment partner (For example: Frederick Gymnastics, 24/7 Dance, Let There Be Rock). Parent Partners MUST register with an enrichment partner that CYM has a partnership with. Please see the Enrichment Partners page for a list of our partners.

ENRICHMENT DISENROLLMENT

If you decide to discontinue an enrichment program, please first inform the Enrichment Partner of your desire to discontinue and disenroll. Please note who you spoke with, the date and time. You will be expected to put that information in the form below. In order to discontinue any class, at least 30 days notice from the first of the month must be given.
